29 CFR  1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:   Trinity Excavating & Demolition at Chatfield Bluffs, specifically at S Kline Street and W Victoria Place Littleton, Colorado 80127: On or about February 8, 2023, employees were exposed to slip, trip, and fall hazards due to an unsafe scaffold structure. The employer failed to ensure planks were at least 18 inches wide.

29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.   a) Trinity Excavating & Demolition at Chatfield Bluffs, specifically at S Kline Street and W Victoria Place Littleton, Colorado 80127: On or about February 8, 2023, and at times prior, employees were exposed to slip, trip, and fall hazards due to unsafe points of access. The employer failed to ensure the scaffold provided a safe means of access. Support boards were utilized as a means of access and the first level was more than two feet above the ground for employees to step up onto, and a free-standing plank was used by employees to access the scaffold on the south side of the scaffold.

The employer failed to ensure a competent person was working on site and qualified to inspect the scaffold.

29 CFR  1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person.   Trinity Excavating & Demolition at Chatfield Bluffs, specifically at S Kline Street and W Victoria Place Littleton, Colorado 80127: On or about February 8, 2023, employees were exposed to slip, trip, and fall hazards due to an unsafe scaffold structure. The employer failed to ensure a competent person was working on site and qualified to inspect the scaffold. The superintendent did not ensure the scaffold was erected in a way that was hazardous to employees and the superintendent did not take prompt corrective measures to eliminate the hazards.
