29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a.) PK100 - On or about February 5, 2023, the employer failed to ensure that an energy control procedure was developed and documented for the control of hazardous energy of the Vortex slide gate valve, Serial No. V159671-061314, Model QC08. An employee sustained two fingertip amputations while cleaning the sifter discharge chute when the employee's right hand came in contact with the Vortex slide gate valve.
