Employees were exposed to the following hazard(s): a) Five roofing employees were actively installing new roofing materials on a residential structure approximately 18 feet above the ground without utilizing a proper means of fall protection.

Employees were observed actively installing new roofing materials, while working from a 10-12 pitch roof approximately 18 feet above the ground.
