29 CFR 1928.57(a)(6): At the time of initial assignment and at least annually thereafter, the employer did not instruct every employee in the safe operation and servicing of all covered equipment with which he was or would be involved, including at least the safe operating practices described in subparagraphs (i) through (v) of this paragraph: (a) The employer did not develop, implement, and train employees on procedures to safely perform work, including the safe disconnect, lock out, and/or block off for all powered equipment, including grain-moving equipment such as but not limited to augers. (b) The employer did not develop, implement, and train employees on procedures to ensure all guards are in place when machines, including augers are operating. This included U-shaped trough augers, belt(s) and pulleys exposing employees to serious injuries . NOTE: Certification of corrective action and supporting documentation are required for this item.
