29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: Workers were exposed to electrical shock hazards at a worksite at 301 Towne Center Drive in Belton, MO 64012. This was last demonstrated on or about August 22, 2022, when an extension cord was being used by employees to power tools and equipment. The extension cord was plugged into another three-outlet extension cord in series. The use of the cords while plugged into another set of extension cords in series is not in accordance with the listing, labeling, and certification of the extension cords.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: Workers were exposed to electrical shock hazards at a worksite at 301 Towne Center Drive in Belton, MO 64012. This was last demonstrated on or about August 22, 2022, when workers were engaged in farming activities. A flexible cord and cable (extension cord) presented with strain relief exposing the internal wiring and was being used by employees to power tools and equipment.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest system, nor were an employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): Workers were exposed to fall hazards at a worksite at 301 Towne Center Drive in Belton, MO 64012. This was last demonstrated on or about August 22, 2022, when workers were engaged in farming activities. a) Workers framing and applying framing to building #8 were exposed to fall hazards approximately 10 feet from the eave of the roof to the ground below. No guard rails, safety net system, or personal fall arrest system were utilized to prevent falls. b) Workers framing and applying joists to building #12 were exposed to fall hazards approximately 10 feet from the eave of the roof to the ground below. No guard rails, safety net system, or personal fall arrest system were utilized to prevent falls. c) Workers framing and applying roof decking to building #23 were exposed to fall hazards approximately 30 feet from the eave of the roof to the ground below. No guard rails, safety net system, or personal fall arrest system were utilized to prevent falls.
