29 CFR  1910.133(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a. On or about March 3, 2023, in the housekeeping area in the basement, the employer did not ensure housekeeping employees were provided with eye protection required by the cleaning chemicals safety data sheets while performing tasks such as filling spray bottles with cleaning chemicals from an Ecolab cleaning chemicals dispensing system.

29 CFR  1910.1030(g)(2)(i):The employer shall train each employee with occupational exposure in accordance with the requirements of this section. Such training must be provided at no cost to the employee and during working hours. The employer shall institute a training program and ensure employee participation in the program.  a. On or about February 9, 2023, a housekeeping employee who had potential occupational exposure to bloodborne pathogens while cleaning blood from the exterior wall of the workplace building had not received bloodborne pathogen training.

29 CFR  1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.   a. On or about February 13, 2023, the employer did not ensure that the personal protective equipment hazard assessment had been performed through a written certification for workplace hazards.

29 CFR  1910.1030(c)(1)(iv):The Exposure Control Plan shall be reviewed and updated at least annually and whenever necessary to reflect new or modified tasks and procedures which affect occupational exposure and to reflect new or revised employee positions with occupational exposure. The review and update of such plans shall also:  a.
