29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled: (a) Location - Middlesex Essex Processing and Distribution Center (MEPDC): On or about 2/15/2023, an employee was operating a forklift with an unstable load of stacked cardboard boxes when it subsequently fell onto a bystander causing crush injuries.
