29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: (a) On or about February 6, 2023, employees were exposed to injuries from falls, while installing metal roof decking on a commercial building without the benefit of fall protection systems being in place.

29 CFR 1926.761(b): The employer did not train each employee exposed to a fall hazard in accordance with the requirements of 29 CFR 1926.761: (a) On or about February 6, 2023, employees exposed to falls hazards while performing steel erection activities had not been provided with training in the recognition and prevention of fall hazards while installing metal decking panels on the roof of a commercial building in excess of 15 feet to the lower working surface.
