29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end.  a) Building Exterior: Employees were exposed to a 22 foot fall hazard while engaged in siding activities from a pump jack scaffold that was 24 feet long. The end of the platform was not designed and installed so that the cantilevered portion of the platform was able to support employees without tipping, the  employees extended the platform with a scaffold plank. The scaffold plank had a 6 foot overhang over the support, on or about 2/13/23.

Employees were climbing the scaffold frames for access to the third level, on or about 2/13/23

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  a) Building Exterior right side: Employees were exposed to a 22 foot fall while engaged in siding activities from a pump Jack scaffold, without utilizing end rails or personal fall arrest system, on or about 2/13/23.  b) Building Exterior front left side: An employee was exposed to fall hazard of approximately  20 fee while working on a pipe scaffold without the use of personal fall arrest system  or other means of fall protection in place, on or about 2/13/23

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.  a) Building Exterior - Roof: An employee was exposed to a 20 foot fall hazard while using anchorage point that was being use to support 2 employees, on or about 2/13/23. The anchorage bracket is design to support only 1 employee.

29 CFR 1926.1053(b)(12): Ladders used where the employee or the ladder could contact exposed energized electrical equipment did not have nonconductive side rails:   VIJAL CONTRACTORS LLC does not ensure that the employee or the ladder could contact exposed energized electrical equipment had non conductive side rails. This violation was most recently observed, on or about 2/13/23, at 231 Hamilton Place Hackensack NJ, at instance   a) Two Story Single Family  Home (Exterior right side): An employee engaged in roofing work was working within less than three feet from energized power lines while working from a Werner aluminum extension ladder that did not had nonconductive side rails. The employer did not ensure that rails of a ladder used near energized power lines were nonconductive, thus exposing employees to an electrocution hazard.
