29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails were not extended at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder: a) Job site, 6529 Ohio River Boulevard, Avalon, PA: On or about February 10, 2023, during gutter repair activities, the employer did not ensure that an employee properly extended a ladder above the landing in that it was only extended above the roofline approximately 1 foot, exposing the employee to a fall hazard of up to approximately 25 feet.
