29 CFR 1910.29(b)(3): The employer did not ensure that guardrail systems are capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied in a downward or outward direction within 2 inches (5 cm) of the top edge, at any point along the top rail. a) Jobsite located at 7825 Holstein Avenue Philadelphia, PA 19153: On or about February 10, 2023, the employer did not ensure that the guardrail system on a mezzanine in the small WAM area was capable of withstanding 200 pounds of force exposing employees to fall hazards. The guardrail failed causing an employee to fall 8 feet down to the lower level.
