29 CFR 1926.1053(b)(1):When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access: 22-28 Mulberry St., Worcester, MA: On or about February 13, 2023, an employee was observed using an extension ladder to reach the top of a steel arch to perform his work, and the ladder did not extend the required 3 feet past that upper landing surface.

1926.760(b)(3) Each connector working at heights over 15 and up to 30 feet above a lower level, was not provided with a personal fall arrest system, positioning device system or fall restraint system and wear the equipment necessary to be able to be tied off; or was not provided with other means of protection from fall hazards in accordance with paragraph (a)(1) of this section. 22-28 Mulberry St., Worcester, MA: On or about February 13, 2023, an employee was engaged in steel work more than 21 feet above the ground below without any fall protection being made available to that employee.
