29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) At the conventional machine area of the Industrial Specialty Co., Inc. Montgomery, AL: On or about February 10, 2023, and at times prior thereto, the employer exposed employees to amputation hazards in that the employees were allowed to operate Harrison Lathe machine without a chuck guard. (b) At the conventional machine area of the Industrial Specialty Co., Inc. Montgomery, AL: On or about February 10, 2023, and at times prior thereto, the employer exposed employees to amputation hazards in that the employees were allowed to operate Cadillac lathe machine without a chuck guard. (c) At the Welding shop: On or about February 10, 2023, and at times prior thereto, the employer exposed employees to crushing hazards in that employer allowed employees to operate the aluminum hanger hook press machine without utilizing one or more guarding method to protect the employees from the point of operation.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasion, and openings through which conductors entered were not effectively closed. (a) At the welding shop: On or about February 10, 2023, and at times prior thereto, the employer failed to ensure the conductors entering the 120V control panel of the press machine were protected from abrasion.
