29 CFR 1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. At 69 Broad Street, Warm Springs, GA 31830 on or about February 8, 2023 and at times prior, the employer exposed employees to fall hazards of over 13 feet 6 inches, in that fall protection was not provided to employees while re-skinning and recovering the roof.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: At 69 Broad Street, Warm Springs, GA 31830 on or about February 8, 2023 and at times prior, the employer exposed employees to fall hazards of over 13 feet 6 inches, in that the extension ladder accessing the roof did not extend 3 feet (.9 m) above the level of the roof surface being accessed by employees while re-skinning and recovering the roof.
