29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns.  a) Jobsite located at 255 Mount Pleasant Avenue, Providence, RI 02908: On or about 2/14/2023, employees were not wearing protective helmets while working below other employees that were working with tools and solar panel equipment on the roof, overhead.  b) Jobsite located at 255 Mount Pleasant Avenue, Providence, RI 02908: On or about 2/14/2023, two employees descended a ladder together, while holding various tools and objects in their hands, with the lower employee not wearing a protective helmet.
