Forklift model #PE4500-60, Ser #10004973 was operated at the facility with a damaged and unreadable warning label.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): (a) On or before 2/14/2023 the employer did not ensure that forklifts were taken out of service and repaired in that powered industrial truck model #PE4500-60, serial number 10004973 was operated at the facility when the back-up alarm was inoperable. This condition exposed employees to struck-by hazards. (b) On or before 2/14/2023 the employer did not ensure that forklifts were taken out of service and repaired in that the Raymond Stand-Up model # 425-C40QM, serial number 425-11-32884 was operated at the facility when the headlights and a yellow strobe warning light were inoperable. This condition exposed employees to struck-by hazards.
