On or about March 30, 2023, authorized employees were manually unjamming pouch machine number 803 at the point of operation multiple times daily. The LOTO procedures were not utilized for the control of potentially hazardous energy including electrical, pneumatic and thermal. An employee suffered a multi-finger amputation.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees: a) At the facility, 625 Fisher Run, Victor, NY 14564, Building 2 - Pouch Machine Area: On or about 3/30/2023, Honeywell Microswitch box for the pull-cord operated emergency stop (e-stop) for Pouch Machine 803 did not have a threaded fastener to secure mounting, exposing the operator to live parts. b) At the facility, 625 Fisher Run, Victor, NY 14564, Building 2 - Pouch Machine Area: On or about 3/30/2023, Videojet Data Flex time stamp for Pouch Machine 803 had a damaged cord, exposing live parts. c) At the facility, 625 Fisher Run, Victor, NY 14564, Building 2 - Pouch Machine Area: On or about 4/4/2023, Honeywell Microswitch box for the pull-cord operated e-stop at Pouch Machine 809 did not have a threaded fastener to secure mounting, exposing live parts. d) At the facility, 625 Fisher Run, Victor, NY 14564, Building 2 - Pouch Machine Area: On or about 4/4/2023, the excess material collection station had a damage conduit connector and missing insulation over live parts. Operators feet are three inches from live parts while walking and observing the pouch machine process.
