OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees operating  a powered industrial truck while working from a platform were exposed to fall hazards:   On or about February 8, 2023, and at times prior, employees were exposed to fall hazards of 4.3 feet to a lower level when working from a platform which was raised by a forklift and the employer did not ensure employees were trained to recognize fall hazards and restraining means such as a guard rail or a means for securing personnel was not provided and used in accordance with ANSI B56.1-2012, American National Standard For Low Lift and  High Lift Trucks.
