29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: On or about February 9, 2023, the employer failed to ensure that the operators of the Core Mold Machines, including but not limited to, Core Mold Machine #U150, were trained as authorized in lock out tag out. Employees are making adjustments to the machines flame and pneumatics while the machine is in operation. There is no interlock on the safety rail/gate or other means in place to control hazardous energy during this process. This task does not meet the minor servicing and repair exemption thus requiring the application of lock out tag out.

Employees are exposed to struck by and crushing injuries during rotation/movement of the machine.

29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about February 9, 2023, the employer failed to ensure that employees were protected from movement of the Core Mold Machine #U150 during its operation. An existing railing was inadequate to prevent exposure during the rotation and tilting of the machine.
