On or about February 15, 2023, during the removal/installation of roofing material at a job site located at 102 Canal St N., Canal Fulton Ohio 44614; the employer did not ensure employees working on a roof of a commercial facility were protected from a fall of approximately 13.2 feet by utilizing a fall protection system.
