On or about February 14, 2023 at the address of 7625 W. Central Ave. Toledo, OH 43617. The employer did not ensure that each powered industrial truck operator successfully completed training and evaluation, prior to permitting employee(s) to operate powered industrial trucks (green Sunbelt rough terrain forklift PIN# JCB5CG7GCM2995545). Without the aforementioned training and evaluation, PIV operators, as well as others working in and around forklift operating areas were exposed to struck-by injuries.

29 CFR 1926.451(e)(8): Direct access to or from another surface was used when the scaffold was more than 14 inches (36 cm) horizontally and/or 24 inches (61 cm) vertically from the other surface: a) Miguel Alfredo Jurado Hernandez DBA Hard Steel LLC. On or about February 14, 2023, at the address of 7625 W. Central Ave. Toledo, OH 43617. The employer failed to ensure a safe means of access/egress was provided to employees. During steel erection activities, employees were utilizing a scissor lift Model# GS-1930 , Serial# 1039795 to gain access to an elevated work area. Employees were required to step more than 42 inches vertical from the scaffolding platform to access the working area. Employees operating in this condition were exposed to falls of 20 feet to the ground below.

On or about February 14, 2023 at the address of 7625 W. Central Ave. Toledo, OH 43617. The employer did not ensure employees were protected from fall hazards while working atop a steel frame structure. Three employees performing steel erection work activities (screwing down decking/seal seams joints) were exposed to fall hazards of 20 feet to the ground below.
