29 CFR 1926.651(c)(2): A stairway, ladder, ramp, or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in-depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: At a worksite located at 306 NW Hwy 7, Blue Springs, MO 64104: Workers were exposed to caught-in and crushing hazards. This was last demonstrated on or about November 22, 2022. Workers were replacing or repairing a broken water line and were not provided any means of egress in an excavation 4 feet or more in-depth. Workers were working inside the excavation without a means of safe egress.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary. At a worksite located at 306 NW Hwy 7, Blue Springs, MO 64104: Workers were exposed to struck-by and crushing hazards. This was demonstrated on November 22, 2022, when workers in an excavation were not provided with protection from falling materials when those workers were replacing or repairing a broken water line. The excavated materials were not placed at least 2 feet or more from the edge of the excavation.

29 CFR 1926.652(a)(1)(ii): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph 29 CFR 1926.652(b) or 29 CFR 1926.652(c) of this section except when: excavations are less than 5 feet (1.52 m) in depth and examination of the ground by a competent person provides no indication of a potential cave-in. At a worksite located at 306 NW Hwy 7, Blue Springs, MO 64104: Workers were exposed to struck-by and crushing hazards. This was demonstrated on November 22, 2022, when workers were in an unprotected excavation that was 6 feet 7 inches in depth. Workers were working to replace or repair a broken water line and were not provided with any means of benching, sloping, or other engineered protective systems.
