(a) Crane CR02; On or about February 13, 2023, employees moved 4,900 pound, 40 foot long steel I-beams through the bay. The I-beams were lifted over the legs and feet of employees working in the area.

(a) Crane CR02; On or about February 13, 2023, The load was not well secured as required as one of the two clamps positioned on the beam did not maintain its hold and became dislodged from the beam.
