29 CFR  1926.501(b)(13) - Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  Location - Residential Construction Site: 370 Broadway Street, Everett, MA  (a) On or about 2/17/23, an employee doing framing work, walking on outer edge two by sixes above the second floor, was not protected from falling to a lower level while working at heights greater than 6 feet.  (b) On or about 2/17/23, employees applying finishing tape to a window frame on the first floor, were not protected from falling to a lower level while working at heights greater than 6 feet.  (c) On or about 2/17/23, employees were exposed to fall hazards from a second floor wall opening without any without any fall protection, guarding, or bracing.  (d) On or about 2/17/23, employees were exposed to a trip hazard due to a hole in the 1st floor subfloor not being covered.

29 CFR  1926.1052(c)(1) - Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stair rail system along each unprotected side or edge:  Location - Residential Construction Site: 370 Broadway Street, Everett, MA  On or about 2/17/23, employees were exposed to fall hazards from a stairway system leading from the 1st floor to the second without a railing installed.
