29 CFR 1926.1053(b)(13):The top or step of a stepladder was used as a step. On or about February 16, 2023, an employee used the top step of a 6-foot A-frame ladder to install electrical wiring inside an existing commercial building under remodeling. The employee was exposed to a 6 feet fall hazard.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards. On or about February 16, 2023, an employee used the top step of a 6-foot A-frame ladder to install electrical wiring inside and was not provided ladder safety training and how to recognize and eliminate fall hazards while using ladders.
