29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about February 21, 2023 the employer did not ensure that machine specific procedures for the Wooden Wick production line were developed prior to employees performing service or maintenance on the equipment. Hazardous energies present include but are not limited to pneumatic, electrical, and mechanical. b) On or about February 21, 2023 the employer did not ensure that machine specific procedures for the Press Tea Light production line were utilized when employees performed service and maintenance to the wick insert area. Employees reach behind a plexiglass barrier to unjam or cut fed wicks. Hazardous energies present on the equipment include but are not limited to mechanical, electrical, pneumatic, and hydraulic.

Employees are tasked with service and maintenance of equipment including but not limited to presses, wick machines, and packaging equipment. Hazardous energies include but are not limited to hydraulic, pneumatic, mechanical, and electrical.

29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about February 21, 2022 the employer did not ensure that effective guarding was in place on the Press Tea Light production line. Effective guarding was not in place to protect the employee from rotating wheels, rollers, and wick feeder. b) On or about February 21, 2022 the employer did not ensure that effective guarding was in place on the Press Tea Light hydraulic press. Effective guarding was not in place on the lower section of the output side of the press. Employees are exposed to the pinch point of the lower ram as it travels up to the frame.
