29 CFR 1910.26(d): The employer did not ensure that measures, such as wheel chocks or sand shoes, are used to prevent the transport vehicle (e.g. a truck, semitrailer, trailer, or rail car) on which a dockboard is placed, from moving while employees are on the dockboard: On or about February 21, 2023 and times there after, employees were exposed to serious injuries and near-misses while loading trucks using powered industrial trucks and the employer did not ensure trucks were chocked to keep them from moving away from the dock.

29 CFR 1910.178(m)(7): Brakes were not set and/or wheel blocks were not in place to prevent movement of trucks, trailers, or railroad cars while loading and unloading: On or about February 21, 2023 and times there after, employees operating powered industrial trucks to load and unload trucks were exposed to accidents and injuries from wheel chocks being removed from the trailers while the operator was still performing loading/unloading duties.
