OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to crushing hazards while operating front end loaders without inspecting the equipment to ensure it was safe to use.   On or about,  February 20, 2023, the employer did not ensure that the Caterpillar 924H front-end loader was inspected daily in order to determine that all components, such as but not limited to, quick disconnect coupler and seat belts worked properly and were in good operational condition.
