The 20-ft. Werner aluminum extension ladder used to access the second level of the structure, was use at an angle that the horizontal distance from the top support to the foot of the ladder (approximately 9-feet and 6 inches) was more than one quarter of the working length of the ladder (4-feet approximately), exposing employees to a fall hazard.
