29 CFR  1910.178(m)(5)(iii):Powered industrial truck(s), with the operator dismounted and within 25 feet of the truck still in his view, did not have the load engaging means fully lowered, controls neutralized, and the brakes set to prevent movement:  a) Building exterior back: The operator dismounted the powered industrial truck and did not lowered the blades. The employee was using the forks with a pallet on it as a scaffold system to install bricks, on or about 2/15/23.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  a) Exterior back: An employee working on the first level of the scaffold was exposed to fall hazards of approximately 6-8 feet onto the ground below due to scaffold not being  fully decked or planked, on or about 2/15/23

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:  a) Building left side: Employees were working on a scaffold that did not bear on base plates or muds sill, on or about 2/15/23.  Employees were laying brick form a tubular welded pipe scaffold. The scaffold was one tier height.
