29 CFR 1910.23(b)(10): The employer did not ensure that any ladder with structural or other defects as immediately tagged "Dangerous: Do Not Use" or with similar language in accordance with � 1910.145 and removed from service until repaired in accordance with � 1910.22(d), or replaced.  a) Warehouse- On February 22, 2023 the rolling ladder used by employees had substantial structural damage, including damaged front wheels, several bent rungs, and had not been removed from service.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR  1910.303(b)(2):Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a) Warehouse - On February 22, 2023 an orange extension cord was connected in series to supply to a power strip which was powering the Uline plastic bag sealer.  b)  Warehouse Dock Area- On February 22, 2023 a electrical junction boxes near the dock door, which are intended to be mounted to a wall and wired with permanent conduit, were not mounted to a wall or fixed location.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
