29 CFR 1910.101(b):The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: a) Maintenance shop area at 27505 Otero Ave, La Junta, CO 81050: Employees were exposed to struck-by hazards due to a gas cylinder not being secured. The employer failed to ensure a gas cylinder was properly secure so that the cylinder did not have a tip over risk.

29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Mechanic shop area of 27505 Otero Ave, La Junta, CO 81050: On or about February 21, 2023, and at times prior, employees were exposed to flying particles and rotating parts when operating the Bridgeport Machines, Inc. Drill Press (Serial # J-68848) in the mechanic shop area. The employer failed to ensure employees were protected from fly-by hazards due to a lack of face shields and guards on the drill press.
