29 CFR  1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:   On or about February 21, 2023, during the installation of framing material at the job site located at 3444 Birch LN, Fairlawn Ohio 44333; the employer did not ensure employees working at an elevated height standing on material resting on the forks of a GEHL Construction Telehandler (All Terrain Forklift) were protected from om fall hazards.

29 CFR  1926.1053(b)(13):The top or top step of a stepladder was used as a step:  On or about February 21, 2023, during the installation of framing material at a job site located at 3444 Birch LN, Fairlawn Ohio 44333; during the installation of framing material on a residential structural, the employer did not ensure employees utilized a stepladder properly as employees utilized the top step of a step ladder to gain access to an elevated working height.

29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about February 21, 2023, during the installation of framing material at the job site located at 3444 Birch LN, Fairlawn Ohio 44333; the employer did not ensure employees working on the roof of a single-family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material.  RJS Builders LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.102(a)(1) Eye and face protection), which was contained in OSHA inspection number 1565600, citation number 01, item number 01 and was affirmed as a final order on 12/30/2021, with respect to a workplace located at 701 Granger Rd, Medina Ohio 44256.

29 CFR  1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.  On or about February 21, 2023, during the installation of framing material at a job site located at 3444 Birch LN, Fairlawn Ohio 44333; the employer did not ensure employees working on a roof of a single-family home were protected from a fall of approximately 19.9 feet by utilizing a fall protection system.   RJS Builders LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13) Duty to have fall protection), which was contained in OSHA inspection number 1565600, citation number 01, item number 02 and was affirmed as a final order on 12/30/2021, with respect to a workplace located at 701 Granger Rd, Medina Ohio 44256.
