29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to the work environment to control or eliminate any hazards or other exposure to illness or injury:  a) Jobsite: On February 22, 2023, an employee installing a fire protection water line, worked in a 5.7-foot-deep area of a trench without any cave-in protective measures used. The employee and the responsible person on site did not recognize the hazard or take action to correct or minimize the hazard.   Note) Certification of corrective action is required for this item.
