29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  a) New residential construction located on Lot #8 at 6230 Nammon Wallace Drive, Cumming, GA 30028, Roof: On or about 02/22/2023, the employer exposed employees to struck-by hazards while operating pneumatic roofing nail guns, in that protective eyewear was not provided to employees operating pneumatic nail guns at 160 PSI.

29 CFR  1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502.  a) New residential construction located on Lot #8 at 6230 Nammon Wallace Drive, Cumming, GA 30028, Roof: On or about 02/22/2023, the employer exposed employees to fall hazards of approximately 11-feet, 2 inches to the lower level, in that a fall protection system was not provided to employees engaged in roofing installation activities.

29 CFR  1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:   a) Residential construction located on Lot #8 at 6230 Nammon Wallace Drive, Cumming, GA 30028: On or about 02/22/2023, the employer failed to inspect all electrical extension cords used to provide power to the Makita 5007F 7-1/4-inch Circular Saw, in that a black 100-ft. electrical cord was damaged at the strain relief on the female end exposing the internal  insulated energized wires. An inspection of these cords was not performed prior to the shift as these cords were not repaired or removed from service.
