29 CFR 1910.22(a)(3): Walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice: On or about March 7, 2023, at the facility located at 410 W Franklin Street, Appleton, WI, employees loading and unloading their vehicles at the back dock were stepping over the elevated metal edge of the inoperative dock plate. The elevated three inch metal edge created a tripping hazard to employees while moving parcels and letter trays into or out of their vehicle.

29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling: a) On or about February 23, 2023, at the facility located at 410 W Franklin Street, Appleton, WI, relocatable power taps (power strips and extension cords) were not used in accordance with its UL listing and labeling. The UL listing prohibits relocatable power taps being used in series. An extension cord connected to a power strip for the Customer Service Supervisor desk computer was powered by the electrical 4 plex pendent located above the registry cage. b) On or about February 23, 2023, at the facility located at 410 W Franklin Street, Appleton, WI, relocatable power taps (power strips and extension cords) were not used in accordance with its UL listing and labeling. The UL listing prohibits relocatable power taps being used in series. An extension cord connected to the electrical 4 plex pendent located above the registry cage was connected to another extension cord with three female electrical connections which was connect to three power strips. c) On or about February 23, 2023, at the facility located at 410 W Franklin Street, Appleton, WI, relocatable power taps (power strips and extension cords) were not used in accordance with its UL listing and labeling. The UL listing prohibits relocatable power taps being used in series. An extension cord with three female electrical connections was connected to a second extension cord with three female electrical connections which was connected to a power strip. The series of relocatable power taps was connected to the electrical outlet on Column D4. d) On or about February 23, 2023, at the facility located at 410 W Franklin Street, Appleton, WI, relocatable power taps (power strips and extension cords) were not used in accordance with its UL listing and labeling. The UL listing prohibits relocatable power taps being used in series. An extension cord was connected to an electrical outlet on Column D6 which was connected to a power strip. e) On or about February 23, 2023, at the facility located at 410 W Franklin Street, Appleton, WI, relocatable power taps (power strips and extension cords) were not used in accordance with its UL listing and labeling. The UL listing prohibits relocatable power taps being used in series. A second extension cord was connected to an electrical outlet on Column D6 , which was connected to a power strip.

29 CFR 1910.304(b)(4)(ii)(B): Where connected to a branch circuit supplying two or more receptacles or outlets, a receptacle may not supply a total cord- and plug-connected load in excess of the maximum specified in Table S-4: On or about February 23, 2023, at the facility located at 410 W Franklin Street, Appleton, WI, the potential maximum ampere load of the 11 Zebra Scanner Charging Units and the 2 Honeywell Scanner Charging Units connected to a single outlet of the overhead pendant located above the registry cage, exceeded the Table S-4 Maximum Cord-and-Plug-Connected Load to Receptacle of 16 amperes for a circuit rating of 20 amperes and a receptacle rating of 20 amperes. The total ampere load for the 11 Zebra Scanner Charging Units and the 2 Honeywell Scanner Charging Units is 33.6 amperes.

The two extension cords were connected to power strips for computer equipment used at the supervisors' desks.

The male connector of the initial extension cord was connected to an electrical outlet on Column D4. f) On or about February 23, 2023, at the facility located at 410 W Franklin Street, Appleton, WI, a power strip was connected to a paper shredder and the Carrier Case R22 light unit female connection. g) On or about February 23, 2023, at the facility located at 410 W Franklin Street, Appleton, WI, an extension cord was connected to an electrical outlet on Column D6 and to a power strip for computer equipment on a nearby Supervisor desk. h) On or about February 23, 2023, at the facility located at 410 W Franklin Street, Appleton, WI, a second extension cord was connected to an electrical outlet on Column D6 and to a power strip for computer equipment on a nearby Supervisor desk. The U. S. Postal Service was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.305(g)(1)(iv)(A), which was contained in OSHA inspection number 1413233, citation number 1, item number 1c and was affirmed as a final order on October 7, 2019, with respect to a workplace located at 13758 US Highway 8, Tripoli, WI.
