29 CFR  1910.28(b)(1)(i)(A): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by a standard guardrail system.  a. The American Steel Products Co. - Toledo, Ohio: On or about February 16, 2023, employees were exposed to fall hazards of approximately 13 feet when working from the cab of the 5-Ton Manning, Maxwell & Moore, Inc. overhead gantry crane. The employer did not ensure the crane cab was equipped with a standard guardrail to protect against falls.   b. The American Steel Products Co. - Toledo, Ohio: On or about February 13, 2023, employees were exposed to fall hazards of approximately 13 feet in when working from the cab of the 20-Ton "riveted construction crane," located in the storage and crane building.  The employer did not ensure the crane cab was equipped with a standard guardrail to protect against falls.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR  1910.28(b)(11)(i): The employer did not ensure that each employee exposed to an unprotected side or edge of a stairway landing that is 4 feet (1.2 m) or more above a lower level is protected by a guardrail or stair rail system.  a. The American Steel Products Co. - Toledo, Ohio: On or about February 16, 2023, employees were exposed to fall hazards of up to approximately ten feet in that the stairway, used to access the 5-Ton Manning, Maxwell & Moore, Inc. overhead gantry crane in the fabrication room, was not equipped with a standard stair rail that included a mid-rail.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

The American Steel Products Co. - Toledo, Ohio: On or about February 23, 2023, the employer did not ensure that electrical equipment of the overhead gantry crane, a 20-Ton "riveted construction crane," located in the storage and crane building, was protected from water and moisture.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. The American Steel Products Co. - Toledo, Ohio: On or about February 13, 2023 the employer did not assure that the overhead gantry crane, a 20-Ton "riveted construction crane," located in the storage and crane building, was thoroughly inspected for deficiencies at least annually.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
