29 CFR 1910.30(a)(1): (a) Fall hazards. (1) Before any employee is exposed to a fall hazard, the employer must provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart. Employers must ensure employees are trained in the requirements of this paragraph on or before May 17, 2017. a. On or about February 23, 2023 at the worksite located at 1015 Main St. Mukwonago, WI 53149, employees were not adequately trained on fall hazards associated with open service pits, this exposed employees to a fall of approximately 4 feet or more.

NOTE TO PARAGRAPH (b)(8)(ii) OF THIS SECTION: When two or more pits in a common area are not more than 15 feet (4.5m) apart, placing contrasting floor markings at least 6 feet (1.8 m) from the pit edge around the entire area of the pits will meet the requirements of paragraph (b)(8)(ii) of this section. a. On or about February 23, 2023 at the worksite located at 1015 Main St. Mukwonago, WI 53149, the employer did not post signage warning employees of fall hazards related to open service pits.
