29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: On or about February 9, 2023, and at times prior to, an employee's finger was amputated due to a crushing injury in December 2022. The employer learned of the amputation on February 15, 2023, and did not report the amputation until February 22, 2023.
