29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). a) On or about February 23, 2023, during the installation of a riser and connection of a sewer pipe at a worksite located at 9606 Lamont Ave, Cleveland, Ohio 44114; the employer did not ensure employees working inside a trench, with a varying depth of approximately 4.7 - 8 feet, were protected from cave-ins by an adequately designed protection system.
