29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   On or about February 22. 2023, and at times prior thereto, employees were framing a new home under construction without the use of hard hats while work was occurring directly above, exposing the employees to the hazard of being struck by falling debris, tools, and materials.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction 6 feet (1.8 m) or more above lower levels were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems nor was an alternative fall protection plan which meets the requirements of 1926.502(k) developed and implemented.  On or about February 22, 2023, and at times prior thereto, employees were framing a new residential home under construction without the use of a fall protection system, exposing the employees to the hazard of falling approximately 25 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  On or about February 22, 2023, several employees were using a ladder to climb from the ground to the roof of a home under new construction and the ladder side rails were not extended at least three feet above the lip of the roof, exposing employees to fall hazards.
