29 CFR  1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  a) NITROcrete, 2040 SE Frontage Road, Fort Collins, CO: On or about February 23, 2022, and at times prior, employees were exposed to respiratory hazards while welding to repair equipment. The employer did not provide medical evaluations to employees required to use the respirators as required in 1910.134(c)(1)(ii).  b) NITROcrete, 2040 SE Frontage Road, Fort Collins, CO: On or about February 23, 2023, and at times prior, employees were exposed to respiratory hazards while welding to repair equipment.  The employer did not provide fit testing procedures for tight-fitting respirators to employees required to use the respirators as required in 1910.134(c)(1)(iii).  c) NITROcrete, 2040 SE Frontage Road, Fort Collins, CO: On or about February 23, 2023, and at times prior, employees were exposed to respiratory hazards while welding to repair equipment.  The employer did not provide procedures for proper use of respirators in routine and reasonably foreseeable emergency situations for tight-fitting respirators to employees required to use the respirators as required in 1910.134(c)(1)(iv).  d) NITROcrete, 2040 SE Frontage Road, Fort Collins, CO: On or about February 23, 2023, and at times prior,, employees were exposed to respiratory hazards while welding to repair equipment.   The employer did not provide procedures for cleaning, disinfecting, storing, inspecting, repairing, discarding, and otherwise maintaining respirators of employees required to use the respirators as required in 1910.134(c)(1)(v).  e) NITROcrete, 2040 SE Frontage Road, Fort Collins, CO: On or about February 23, 2023, and at times prior, employees were exposed to respiratory hazards while welding to repair equipment.  The employer did not provide training of employees in the respiratory hazards to which they are potentially exposed during routine and emergency situations for employees required to use the respirators as required in 1910.134(c)(1)(vii).  f) NITROcrete, 2040 SE Frontage Road, Fort Collins, CO: On or about February 23, 2023, and at times prior, and at times prior, employees were exposed to respiratory hazards while welding to repair equipment.   The employer did not provide training on putting on, removing, or the limitations of the respirators for employees required to use the respirators as required in 1910.134(c)(1)(viii).  g) NITROcrete, 2040 SE Frontage Road, Fort Collins, CO: On or about February 23, 2023, and at times prior, employees were exposed to respiratory hazards while welding to repair equipment.   The employer did not establish work-site specific procedures to regularly evaluate the effectiveness of the program as required in 1910.134(c)(1)(ix).

29 CFR  1910.179(j)(2)(iv): Monthly inspections of hoist chains, with a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected, were not performed:   a) NITROcrete, 2040 SE Frontage Rd, Fort Collins, CO: On or about February 23, 2023, and at times prior, employees were exposed to struck-by and crushing hazards associated with a gantry crane utilizing chains that had not been adequately inspected on a monthly basis. The employer failed to conduct monthly safety checks of chains used to suspend loads up to 2,000 lbs.
