(a) This violation occurred on or about February 23, 2023, at a job site located at 5728 Huffines Blvd, Royse City, TX, where employees installing Therma heath siding insulation on a two stories single-family house while working from a pump jack scaffolding system and were not protected from a fall hazard: JUAN GUADALUPE RESENDIZ WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 1926.451 (G) (1) OR ITS EQUIVALENT WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1626722, CITATION NUMBER 1, ITEM 1 THAT WAS ISSUED ON OCTOBER 13, 2022, WITH RESPECT TO A WORKPLACE LOCATED AT 2805 PRAIRIE OAK STREET, MANSFIELD, TX 76063. THE FINAL ORDER DATE WAS DECEMBER 9, 2022.
