29 CFR  1910.178(q)(1):Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel:  Employees were exposed to struck-by hazards.  Employees engaged in material handing activities using the Toyota forklift, serial number 75280, were exposed to struck-by hazards in that forklift was not taken out of service when it was found to be unsafe and in need of repair.  The horn and lighting to indicate the forklift is in reverse was non-operational upon evaluation of the forklift that was considered in-service.
