29 CFR 1926.451(b)(1)(i):Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary. (a) On or about February 23, 2023, at 1299 South Goodman Road, Champions Gate, FL: employees were exposed to fall hazards of approximately 6 feet 6 inches in that the spacing in between the individual walk planks and the spacing between the platform and the uprights was approximately 2.5 inches.

(a) On or about February 23, 2023, at 1299 South Goodman Road, Champions Gate, FL. employees were exposed to fall hazards of approximately 13 feet 3 inches in that the scaffold guardrail system was erected in such a way that it was not capable of supporting at least 200 pounds.
