29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: a) Worksite: On or about February 24, 2023 employees were not protected from fall hazards on all open sides and ends while installing siding on the Community Building, due to lack of a mid-rail and end-rails while working from a height in excess of 14 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Community Building: On or about February 24, 2023, employees were not protected from falling and/or tripping hazards while using a ladder, that did not extend at least three feet above the upper landing surface, to access the pump jack scaffold platform. The working platform was fourteen feet (14') above the ground and the extension ladder only extended to sixteen feet (16') above the ground.
