29 CFR  1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   On or about February 23, 2023, and times prior thereto, employees were exposed to struck-by hazards while using portable tools such as, but not limited to,  circular saws without any type of eye protection.

29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:   On or about February 23, 2023, and times prior thereto, employees performing masonry work on the second level of a scaffold were not provided a ladder for access.

29 CFR  1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:  On or about February 23, 2023, and times prior thereto, employees were exposed to fall hazards while doing masonry work at heights greater than 10 feet from the second level of a scaffold without any type of guardrails.
