Mr. Wilmer Joel Cruz Mejia was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1497251, citation number 1, item number 1 and was affirmed as a final order on 11/30/2020, with respect to a workplace located at 240 Lakeside Dr., Odenville, AL.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  (a) On or about 02/22/2023 - 60 Clubhouse Circle, Odenville, AL, (Front of house), a roofer removing old roof shingles while using a mountain climbing belt for fall protection in lieu of appropriate personal fall arrest system was exposed to a fall hazard of approximately 14 feet, 5 inches.    (b) On or about 02/22/2023 - 60 Clubhouse Circle, Odenville, AL, (Left side of house), roofers placing shingles using mountain climbing belts for fall protection in lieu of appropriate personal fall arrest system were exposed to a fall hazard of approximately 13 feet, 4 inches.  Mr. Wilmer Joel Cruz Mejia was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.502(d)(17), which was contained in OSHA inspection number 1497251, citation number 1, item number 2 and was affirmed as a final order on 11/30/2020, with respect to a workplace located at 240 Lakeside Dr., Odenville, AL.
