29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems or personal fall arrest systems. On or about February 23, 2023, one employee was performing masonry operations from the roof of a new commercial building from a height of approximately 20 feet from the ground without using fall protection, exposing the employee to fall hazards.

29 CFR 1926.1053(b)(4):Ladders were not used for the purpose for which they were designed. On or about February 23, 2023, one employee was performing masonry operations from the roof of a new commercial building using an A-frame portable ladder in the closed position as a means to go up and down from different roof line elevations, exposing the employee to fall hazards.
