29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about February 23, 2023, during the installation of framing material at a job site located at 2131 Hawthorn Ct, N. Canton Ohio 44720; during the installation of framing material, the employer did not ensure employees working below the elevated carpenter was protected from possible danger of head injury from impact, or from falling or flying objects, falling debris from working surfaces above his location.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about February 23, 2023, during the installation of framing material at a job site located at 2131 Hawthorn Ct, N. Canton Ohio 44720; the employer did not ensure employees working on a roof of a single family home were protected from a fall of approximately 10.4 feet by utilizing a fall protection system.
